If the Offensive Formation is suppose to "key" what the Defense is suppose to line up in, then why would a team line up in Goal Line Attack #3 on 1st & 15 (good distance for a possible pass) when the offense comes out in Shotgun Normal Double Outs, (a short passing formation). According to the card, this play is in my playbook with a 98% familiarity on Offense and 100% Familiarity on Defense.
