I think the reason is my current strategy. I'm trying to find the best plays for my team regardless of BigData. I found out that there are several different good working plays for different teams or HCs. So most of my teams (I know, I have too many) are in play finding mode. All matches will be different next season ;-)

Orlando called a lot of zone and zone blitz defensive plays. Zone combined with overuse if I had to guess was how it happened
One thing which makes me wonder: I've had a set of 30 different defensive plays and no rules active. Why did my coaching staff call the same play so often that there appeared an overuse penalty???

The only thing then would be your Defense sliders and if your play calling isn't matching up close to your sliders then it would have to be random chance

Honestly, I don't know. Perhaps it's the sliders like AG said? I ran 32 different offensive plays, so it's not like I ran the same play on offense which would put you in the same defensive play.
