I wouldn't get hung up on this. The height generation is off for all positions, and height doesn't control much right now (it's apparent it's not used in Frame either).

Players just standing there at the LOS are usually a result of broken B&R interaction. LBs covering RBs out of the backfield -- I'm almost certain -- rely on SP more than coverage to track the player because they are moving laterally to the LOS. Only once the RB breaks the LOS does coverage become important, but yes, in general, the response time is muted in 0.4.2. It's a little better in 0.4.3, but some of that is about how quickly the QB makes the decision to throw.

No idea. I don't use plays where the safety is in Deepest Man. They have never seemed to work right, so I have never bothered looking closely at them.

Also not sure since I haven't seen this happen enough to know. I wouldn't be shocked if the B&R roll is happening for both players at the same time.
