This is correct. You can see what each player on your team knows about your playbook in his "plays" tab.

The players study all the plays in your coach's playbook during training camp, then they study the plays you have selected for the game plan during each game week. They also gain some knowledge of the play every time they are part of the play during a game. If you have a similar game plan every week and don't have a lot of roster turnover or change your head coach you shouldn't take too long to get high familiarity bars for your most-used plays. Scouting plays usually takes longer because you don't necessarily see the same plays from week to week.

What's just a bit odd, for instance, is how my starting RB the past two seasons (starting as a rookie) has only ~50-60% knowledge of my four most frequently called run plays, all called over 100+ times in that time frame, but has almost perfect knowledge of 9 other run plays that I rarely if ever call.

My understanding is that the Team Familiarity meter is a reflection of the sum PLAYER knowledge, rather than the coach's understanding of the plays in his own playbook. Different coaches (even those with the same style, ie West Coast) will have access to different plays, but won't call plays that you do not 'check off' on.

I dont think this is true. In leagues that I have drafted for allocation draft that are now 3 seasons old the turnover pretty minimal because of initial contracts and I still have most of the same basic plays in them and like I have said the familiarity is about 33% at most and some even lower. I think the gains and losses should be adjusted but also dont think its a top priority either.
